MSHSL Sports Physical Examination Form

2024-2025 MSHSL Physical Requirement

• All students are required to have a current physical exam form on file with the Activities Office in order to participate in MSHSL sports and activities. Please click on the link below to access the form and submit a completed form to the Activities office prior to the first day of the season. 

A current physical can be emailed to (erhs.activities@isd728.org) or faxed to 763-241-3421 in the ERHS Activities Office.
